Side A is for vehicles removed by a public agency for reasons other than abandonment CVC 22851.8, and side B is for vehicles removed by a public agency because of abandonment CVC 22851.8.

You may obtain a photocopy of the report that the other driver submitted to DMV by completing a Financial Responsibility Information Request (SR 19C) form and mailing it to the address printed on top of the form.
