This form is specifically for new nonresident or imported vehicles. This form with the appropriate fees may be submitted to request replacement of the following items that were lost, stolen, destroyed, or mutilated: license plates/sticker, CVRA weight decal, registration card, disabled person placard/disabled person identification card, and Vessel Certificate of Number.

Statement of Facts Motor Carrier Fleet Name Change (REG 256M)

This form is an application to change the name of a Motor Carrier Fleet.

Certification for Disabled Veterans License Plates (REG 256V)

This form is used to certify that one is a Disabled Veteran as defined under CVC §295.7

Disabled Person or Disabled Veteran License Plate Certification Form (REG 3060)

Owners of disabled person or disabled veteran license plates must complete this form and present it to peace officers upon request. The form identifies the owner of the special license plate and verifies that person's right to disabled person/veteran privileges.

Verification of Vehicle (Not to be completed by applicant) (REG 31)

This form is used only by authorized DMV and Auto Club employees, DMV licensed vehicle verifiers, California Highway Patrol (CHP) and other peace officers, when performing a physical inspection of a vehicle or trailer vehicle identification number (VIN) and other vehicle characteristics.

Foreign Resident In-Transit Permit Application (REG 34 DRM)
