Outline of Required Topics and Standards for CA DMV Approved Traffic Violator School Course (OL 613)
You need a traffic violator school (TVS) owner license if you own a TVS that offers traffic safety instruction for individuals referred by a court. A traffic violator school owner license does not authorize you to give instruction.
Initial Requirements (before you apply for a traffic violator school owner license)
Before you apply for your traffic violator school owner license, complete the following steps:
- Request for DMV Approval of a Traffic Violator School Name (OL 612).
- Get lesson plan approval (submit one of the following):
- If creating an original lesson plan, submit a Request for Approval of TVS Educational Program (OL 764), course materials, and fees.
- If using a previously approved lesson plan, submit a Request for Use of an Approved TVS Program (OL 766), letter from the program owner authorizing you to use the lesson plan, and fees.
Mail your form, documents, and fees to:

Allow a minimum of 120 days for a response. Incomplete requests will be returned. Lesson plans will be logged and forwarded for review in the order received.
Fees
- Lesson plan review and approval fees (if creating original lesson plan):
- $475 classroom course review fee
- $800 home study course review fee
- $800 internet course review fee
- $25 lesson plan approval fees (if using previously approved lesson plan).
How to Apply for a TVS Owner License
- Application for Traffic Violator School (TVS) Owner License (OL 713).
- Application for Occupational License Personal History Questionnaire (OL 29B) for each person listed on form OL 760.
- Property Use Verification for a Driving School or Traffic Violator School License (OL 140).
- Traffic Violator School Owner Assurance of Compliance With the Americans with Disabilities Act (ADA) of 1990 (OL 142).
- Traffic Violator School Classroom Lease or Rental Agreement (OL 144).
- Traffic Violator School (TVS) Owner Surety Bond (OL 704 B) or Deposit Agreement and Assignment (OL 25E).
- Traffic Violator School Branch Business Office/Classroom Application (OL 712).
- Live scan fingerprint y Request for Live Scan Service (DMV 8016).
- Fingerprint card (ADM 1316) (out-of-state applicants only). Fingerprint cards may be obtained by calling Occupational Licensing (OL) at (916) 229-3126.
- Appointment of Director as Agent for Service of Process (ADM 9050).
- A completed OL 764 or OL 766 and a letter signed by DMV or an agent of DMV approving your lesson plan or authorizing you to utilize a previously approved lesson plan (see above).
- Evidence of your property ownership or a copy of your rental or lease agreement for all offices and classrooms.
Once you have completed all mandatory forms and collected all required documents, call your local OL Inspections office to set an appointment to submit your paperwork and fees.
After DMV processes your application, you’ll receive an email instructing you to schedule an inspection of your business location. Once you receive that email, you’ll need to create a DMV online account and select a time slot for an inspection. The email you receive will provide the information you need to get started.

The OL inspector will:
- Inspect the main business office where you conduct business.
- Inspect all books and records pertinent to the business.
You must complete your application within one year or a new application with all examinations and fees will be required.

- $200 non-refundable original or reinstatement application fee
- $1 family support program
- $15 duplicate license fee (if applicable)
When applying for any combination of owner/operator/instructor license, only the owner fee will be due.
How to Renew a TVS Owner License
To renew your license, submit the following:
- Driving and Traffic Violator School Owner Renewal Application (OL 45A).
- Classroom/Branch Renewal Application (OL 737).
- If creating an original lesson plan, submit a Request for Approval of TVS Educational Program (OL 764).
- If using a previously approved lesson plan, submit a Request for Use of an Approved TVS Program (OL 766).
